Incident Referral Forms For Social Workers-- DCFS/SASS Referrals

As a social worker or school counselor, our jobs are filled with crisis calls that we alone cannot manage and often have to refer to outside services. It can be difficult to case manage and keep up with the outcomes, which is why this form is perfect! This form allows you to track the type of referral that was made and includes a note section to put the outcomes of the referral.

Any time you do a threat assessment, make a DCFS call, or Refer to outside support for other crisis services, you will track it in this form. Simply put the date the referral was made, the students name, the time the referral was made, check one of the referral categories, and type in any important notes about the case that you would need. (Ex: Called DCFS about John Smith.. Case # 123456789)

This form also helps with end of the year reporting for admin so you can tally exactly how many crisis referrals you may have throughout the year and helps you to case manage in a more organized format! This also helps you to allocate for your time and your program!
