Product description

From the author

This resource is a set of Phonics Progress Monitoring Tracking Forms.

This pack includes 17 different sets of phonics assessment sheets to help you monitor the progress of your students.

This will help you monitor students phonological and phonemic awareness, reading and spelling skills.

This pack helps you collect data to determine if your teaching program or intervention is working, those students who may be struggling with particular skills, and if you need to re-teach some areas.

The sets include 1-, 2- and 3- syllable words for differentiation.

Teaching tips & learning objectives

Learning objectives

  • Identify, blend, segment and manipulate phonemes within words matched to a specific target sound.

  • Read one-, two-, and three-syllable words containing a target sound-spelling pattern.

  • Spell words containing the target sound using dictation and structured spelling tasks.

Teaching tips

  • Conduct the oral and reading assessments one-to-one, but administer the spelling assessment in small groups or as a whole class, per the pack's own recommendation.

  • Pick and choose only the sets or tasks relevant to your current teaching focus rather than working through all 17 in order, since the pack explicitly supports either approach.

  • Use the bracketed correct answers on each oral prompt sheet to mark responses in real time and highlight errors as you go.

Skills covered

  • Identify, blend, segment and manipulate phonemes in words

  • Spell target patterns through structured spelling and dictation tasks

  • Decode one-, two- and three-syllable words

Questions teachers ask about this resource

How does the pack help pinpoint where a student is struggling?

Each set's cover sheet splits scoring into oral, spelling and reading sections with a separate accuracy percentage for each, so a low score can be traced to a specific skill area rather than just an overall total.

Which phonics programs can these assessments be used alongside?

The pack states the assessments can be used with any phonics program, since they are organized by phonics knowledge -- specific sound-spelling sets -- rather than tied to one curriculum's scope and sequence.

Does the PDF calculate scores or include answers?

Teachers calculate and enter the totals and percentages. Oral prompts include bracketed answers; reading and spelling sections provide words and models for checking.
