Student Assistance Team Meeting Record

This is a Student Assistance Team Meeting Record for your Intervention Team. With this record, you can be prepared with student data and services for meetings with families, teachers, teams, and admins. This 2-page template will come equipped with all the pertinent information you’ll need to document.

At the start of the template, there is space for parents and teachers to list their concerns separately. Whoever is filling out the form will have space to write out the strengths and talents of the student. After providing background information and baseline data, you will have an opportunity to write about the interventions that are currently provided. You will be able to make note of scores for any universal screeners, benchmark screeners, and progress monitoring that is taking place. At the end of the meeting, carve out time for the teacher, interventionist, and family to discuss next steps.
