Adult Services Readiness and Referral Tracker for 18-21 Transition

About This Product
This adult services readiness assessment and referral tracker helps students in 18-21 transition programs get ready for adult services and keep track of every referral.
Why I made this: The summer after one of my students aged out, his mom called me from her kitchen table with a stack of letters from offices she had never heard of. I made this so the next student leaves with a plan, a documents folder, and a tracker.
What’s included:
- A 22-page PDF
- A teacher page with exact counts, timing, and how to use the results
- An 8-question "after school" interview in the student’s own words
- A readiness overview for 8 kinds of adult services
- 8 service readiness pages (vocational rehabilitation, developmental disability services, supported employment, day programs, benefits and health coverage, housing support, transportation, and a college disability office), each with what it does, what they may ask, what to bring, and my questions
- A 16-item documents folder checklist and an intake meeting prep page
- A 2-page referral timeline and waitlist tracker and a "my services map"
- A family or guardian input page and a 2-page team summary
How to use it: Start with the interview and the readiness overview. Pick 2 or 3 services and do those pages before the first call. Build the documents folder with the student and family, prep for each intake meeting, and update the tracker after every contact.
Who it’s for: 18-21 program teachers, transition coordinators, case managers, and families of young adults who are getting ready to leave school services.
Format: PDF, US Letter, 22 pages. Prints well in black and white.
