About this resource

What's inside this resource

This student information sheet for parents to fill out is a one-sheet family form, printed front and back, that helps a K–8 teacher get to know each child and reach the right adult from the first week of school. It opens with a short welcome note telling families to share only what they are comfortable with.

Front of the sheet

  • Child's full name and birthday
  • Contact details for two parents or guardians, each with a checklist of the best ways to reach them (email, phone call, text message or a note sent home)
  • Brothers and sisters, with room for three names and ages
  • Home and homework: which tools the child can use at home, how often someone can help, the languages spoken at home and the language the family wants school notes in

Back of the sheet

  • Health and safety: allergies, medical needs and food needs on separate lines, plus a yes-or-no medication question with a small medication, time and dose table
  • Your child, in your words: sentence starters such as "Ask my child about…" and "My child shines when…", and a checklist of how the child learns best
  • This year: anything coming up that could affect the child at school, and space for the family's own questions
  • An optional "My child in three words" box

The form comes in full color and in a print-friendly black and white version. There are no right answers, so no answer key is included.

Teaching tips & learning objectives

Learning objectives

  • Gather accurate contact details and each family's preferred way to be reached before the first message home.

  • Learn about allergies, medical needs and medication at school early enough to share them with the office or nurse.

  • Understand each child's home homework setup, including device and internet access and how often an adult can help.

Teaching tips

  • Send the form home in the first week with a return date written at the top.

  • Copy the black and white version on the school copier so every family gets a form, and keep a few spares for late enrollments.

  • Pass allergy, medical and medication answers to the school nurse or office as soon as forms come back; the form points families to the office for its own medication permission form.

Skills covered

  • Family contact planning: the two guardian blocks record phone numbers, email and the contact method each adult prefers, so teachers know who to reach and how.

  • Health awareness: separate lines for allergies, medical needs and diet, plus a medication table, keep safety details in one place.

  • Home learning context: questions on home devices, internet, printers, homework help and home languages show what support a child has outside school.
