IEP at a Glance Sheets for Secondary + Transition Students

About This Product
These IEP-at-a-glance sheets are one-page snapshots for secondary and transition students: a classroom version, a transition and job-site version, cut-apart para pocket cards, and a related services page.
Why I made this: Gen ed teachers, paras, and subs do not have time to read a full IEP before class, and they should not have to. They need the few things that matter every day. These sheets put that on one page, with a health box to check instead of medical details on paper.
What's included:
• Cover and case manager how-to page
• Student IEP-at-a-Glance (classroom)
• Transition and Job-Site Glance
• Para pocket cards, two per page
• Related services at a glance
• An example classroom glance filled in for a made-up student
How I use it: Pick the right version for the audience. Keep it short. Use first names or initials. Share only with staff who work with the student. Update after every IEP meeting and at the start of each semester. Fill in by hand or type in the Word file.
Who it is for: secondary SPED teachers, case managers, transition coordinators, and paras on grades 6 to 12 and 18-21 transition teams.
What you download: one printable PDF, 7 pages, US Letter, with write-on lines and tables.
Real buildings vary. Fill in your own names, dates, and local rules. Always follow your state and district policies.
Good to know: the sheets are meant to be short on purpose. The health box asks staff to check a box rather than write medical details, and the example page shows how much to include. Print a stack at the start of the semester, fill them in after each IEP meeting, and keep them in a private folder with the rest of your caseload paperwork.
