Read Appointment Cards and Reminder Texts for Health Visits

About This Product
This reading appointment cards and reminder texts set helps students read health visit paperwork: the day, date, time, where to go, what to bring, and how to cancel or reschedule.
Why I made this: A student missed two dentist visits because she thought "R to reschedule" meant "received." Nobody had read those texts with her. This set uses cards and texts that look real, so the next reminder makes sense the first time.
What's included:
• A 13-page PDF and an editable Word copy
• A teacher page with a three-level table and a 2-week plan
• 3 documents: two appointment cards (a clinic and a dentist), a phone screen of reminder texts, and a clinic "before your visit" sheet
• Level A ●: 10 cards with 2 choices, in 16 point type
• Level B ■: 10 cards with 3 choices, including arrival times and cancel deadlines
• Level C ▲: 10 short-answer cards
• 3 practice pages, an answer key, a data sheet with a 10-session graph, a try-it-for-real page, and blank cards
How to use it: Read the cards and texts together. Work one level at a time and check with the codes. Level C can be spoken or written. Then read a real appointment card or reminder with an adult.
Who it's for: High school special education teachers, 18-21 program teachers, job coaches, and families helping students manage their own health appointments.
Format: PDF, US Letter, 13 pages, plus an editable Word copy. Prints well in black and white.
How this is different: Phone Skills for Everyday Calls | Appointments, Voicemail + Phone Menus teaches making the call. Reading an Insurance Card | Member ID, Copays + Who to Call | Life Skills teaches the insurance card. Phone Skills for Everyday Calls has one dental appointment card with a call script. This set is about reading appointment cards, reminder texts, and clinic instructions, with no calls.
Pairs with: Reading an Insurance Card | Member ID, Copays + Who to Call | Life Skills; Phone Skills for Everyday Calls | Appointments, Voicemail + Phone Menus; Social Narrative: Going to the Doctor | Young Adult Health
